You will be joining a deeply experienced team with decades of operating experience in care delivery and healthcare technology. The team is led by Prabhjot Singh, MD, PhD (sold CHW cares to Oak Street Health, senior advisor and co-chair AI task force at Peterson Health Technology Institute) and Abhishek Gupta (ex-Oak St NY market leader, former Zus and MVP Ventures) with Margaret McKenna (former CTO Devoted Health) as board member and tech advisor.

The Role

We’re seeking a Senior Full-Stack Engineer (ideally NYC/NJ based) to lead development of clinician-facing products that sit at the intersection of healthcare, AI, and modern software design. You’ll play a key role in architecting and building the core experiences that clinicians use every day — helping transform complex healthcare workflows into intuitive, intelligent tools.
This role is ideal for someone who thrives in high-ownership environments, brings a strong product sensibility, and is motivated by building technology that directly impacts care delivery. You’ll work closely with product, design, and clinical experts to translate needs from the frontlines of care into scalable, elegant software.
As a senior member of the engineering team, you’ll have broad influence across the stack — from shaping architecture and infrastructure decisions to driving the quality and polish of the clinician experience. You’ll also help define how we build AI-native applications responsibly and effectively, grounding cutting-edge models in real clinical use.
This is an opportunity to join Altitude at a pivotal stage of growth, contributing to the foundation of our product and engineering culture. Reporting to the Head of Engineering, you’ll help define how healthcare software should look, feel, and perform in the AI era.

What You'll Do

Build and scale full-stack features end to end — from responsive front-end interfaces to performant, secure backend systems.
Design elegant, reliable experiences for clinicians and care teams, ensuring usability and trust in high-stakes workflows.
Collaborate cross-functionally with product, design, and clinical experts to translate complex requirements into high-impact software.
Integrate with healthcare data sources and systems (EHR, FHIR, HL7) while maintaining compliance, security, and reliability.
Design and optimize data storage and access layers ensuring data integrity, privacy, and performance across multi-tenant healthcare systems.
Develop and iterate on AI-powered product experiences, applying modern LLMs and context orchestration frameworks.
Uphold engineering best practices — performance, testing, observability, and maintainability at scale.
Contribute to system design and technical direction as the company grows from early product to scale.

What We're Looking For

Proven experience building healthcare SaaS or clinician-facing applications with high reliability and user trust.
Strength in modern front-end frameworks (React, Next.js, TypeScript) and fluency in backend systems (Node.js, Python, or similar).
Experience modeling and managing complex relational data (PostgreSQL, or similar), with a deep understanding of schema design, query optimization, and data lifecycle management in regulated environments.
Strong product intuition — you care about user outcomes, not just tickets.
Deep curiosity about AI-native application design, with hands-on experience integrating LLMs or related infrastructure.
Ability to design scalable, secure architectures and data models that meet healthcare compliance requirements.
Clear communicator who thrives in collaborative, fast-moving environments.
Bonus: experience with healthcare integrations, workflow automation, or clinical decision support tooling.
