Job Summary
The Physical Therapist is a key member of an interdisciplinary care team, responsible for planning, coordinating, and delivering restorative and rehabilitative therapy services. Key Responsibilities
The Physical Therapist is a key member of an interdisciplinary care team, responsible for planning, coordinating, and delivering restorative and rehabilitative therapy services. This role focuses on helping participants achieve maximum independence, safety, and overall functional ability through patient-centered care.
The position emphasizes collaboration across clinical and non-clinical team members to ensure comprehensive and high-quality outcomes.
Key Responsibilities
- Conduct physical therapy assessments in both facility and occasional home settings
- Define participant care needs, goals, and preferences
- Actively participate in interdisciplinary team (IDT) meetings to develop care plans
- Deliver and document therapy interventions based on care plans
- Provide treatments including:
- Active and passive exercise
- Muscle re-education
- Gait training
- Functional training for activities of daily living (ADLs)
- Therapeutic modalities (e.g., heat, light, sound, water, electrical stimulation, massage)
- Supervise Therapy Assistants and provide training for Functional Maintenance Programs (FMP)
- Educate staff, participants, and caregivers on proper use of therapeutic equipment
- Bachelor's degree in Physical Therapy (Master's or Doctorate preferred)
- Valid California Physical Therapy License (or eligibility to obtain)
- Minimum 2 years of experience in a clinical setting with frail or elderly populations preferred
- Experience in outpatient, acute care, inpatient, or skilled nursing settings is preferred
- CPR and First Aid certification required within 180 days of hire (preferred upon hire)
