Position Summary

The Licensed Vocational Nurse (LVN) provides nursing care under the direction of a Registered Nurse and the hospice care plan.

Responsibilities Include

Essential Duties and Responsibilities

• Monitoring patients' conditions, administering medications and treatments, reporting changes in patients' status, and ensuring comfort and dignity for hospice patients.
• LVN collaborates with other health care professionals to meet patient and family needs in a compassionate manner.

• Strong interpersonal and clinical skills
• Ability to follow care plans and document accurately
  
  

Education & Experience

• Current LVN license in the state of practice
• At least one year of experience, preferably in hospice or home care
